Free Transfers
Unlimited-Ride MetroCard permits free transfers

Pay-Per-Ride MetroCard allows one free
transfer of equal or lesser value if you complete your transfer
within two hours of the time you pay your full fare with the same
MetroCard. If you pay your local bus fare with coins, ask for a
free magnetic-strip transfer to use on another local bus.
Reduced-Fare Benefits
You are eligible for reduced-fare
benefits if you are at least 65 years of age or have a qualifying
disability. Benefits are available (except on peak-hour express
buses) with proper identification, including Reduced-Fare
MetroCard or Medicare card (Medicaid cards do not qualify).

Yes, there is a service(minivan) called Connecticut Limo that goes to Hartford every hour from JFK. You can book it at Ground transportation(Welcome Center) counter at each terminal.
The price is $93/passenger. The shuttle drops you off at Holiday Inn at Hartford.

Looks like your best bet is by train to Atlantic City and then local transportation to Surf City. Go to Penn Station by taking Airtrain from your terminal to Jamaica station and either E subway or LIRR (faster) to Penn Station. Take Amtrak (Northeast Corridor) to Philadelphia 30th Street Station and transfer to the Atlantic City train. Take local taxi or public transportation from Atlantic City to Surf City. NJ Transit also has the North Jersey Coast train from Penn Station to Bay Head but that may be too far north from Surf City.

Take Airtrain from any terminal on track 1 to Jamaica Station( be careful - on that track there are two alternating trains going to different routes; check the sign above the doors as the train arrives; the red colored sign is Jamaica one). Then take subway line E to 42nd street stop. Cost-$7.25/passenger.
At 42nd street look for Port Authority Bus Terminal and take Greyhound bus to Hartford. The fare is ~$30.

I am not familiar with Newark Airport , but looking at the map your best bet may be Airtrain to Penn Station Newark and then Amtrak to Baltimore.

Some Amtrak trains also stop at Newark Airport Station so check Amtrak's schedule.

Take Airtrain to Jamaica Station and then subway E to Canal Street stop.
or
Take Airtrain to Howard Beach Station and then subway A to Canal Street stop.
Either way will take you around 1:15-1:30 min
