JFK to anywhere in Manhattan is a flat rate of USD 45 (plus about 4.50 for the motorway "eze-pass"). That's it: not extra charges for luggage, or time of day, etc. Travelling with kids, I'd recommend the ease of the taxi service -- and AVOID any taxi that is not yello: ignore anyone offering/whispering a taxi service: go straight to the official yellow cab taxi dispatch

A friend of mine is visiting the USA from Russia so will be flying in and out of JFK as he tours around. Are there any mobile phone rental companies in the airport, like in many other international airports, where he can rent a phone for 20 days while he travels the USA?

He can get a pay as you go Phone, from At&T. It`s a little expensive, but he gets a phone numer while in the USA.
